Everyone Health Southwark are now opening up a self-referral option for the Healthy Lifestyle Hub during the COVID-19 period.
To be eligible for referral clients must be:
- aged 18 years of age or above,
- Southwark resident
- Or be registered with a GP practice in Southwark
Must have at least one of the following:
- Has a BMI ≥ 30 (or ≥ 27.5 if Indian, Pakistani, Bangladeshi, other Asian or Chinese)
- Is pre diabetic (HbA1c 42-47 mmol/mol)
- Has anxiety and or depression.
- Has a cardiovascular risk of more than 10% using Qrisk2
- Is considered inactive or moderately inactive determined by the GPPAQ for the last 3 months or more.
- Had a cardiac event in the last 12 months.
- A smoker
